We are seeking a dedicated Physical Therapist to join our healthcare team in Bath, Maine. As a vital member of our rehabilitation services, you will provide comprehensive physical therapy to deliver optimal patient outcomes.
  • Conduct thorough patient evaluations and develop individualized treatment plans
  • Implement evidence-based physical therapy interventions and monitor patient progress
  • Document patient assessments, treatment plans, and progress notes with attention to detail
  • Supervise and coordinate care delivered by physical therapy assistants and rehabilitation technicians
  • Establish measurable treatment goals and adjust plans based on patient progress
  • Provide patient and caregiver education regarding treatment plans and home exercise programs
  • Collaborate with referring physicians and other healthcare providers
  • Maintain accurate and timely documentation in compliance with healthcare standards
  • Utilize rehabilitation equipment and techniques effectively
  • Participate in regular team meetings and care coordination discussions
