Physical Therapist - Skilled Nursing

Location: Mount Vernon, New York

Position Overview:

We are seeking an experienced Physical Therapist to join our skilled nursing team. The ideal candidate will have expertise in rehabilitative care within skilled nursing settings, working closely with residents recovering from surgery, illness, or managing chronic conditions.

Salary Range: $100,000 - $120,000 per year

Job Type: Full-Time

Schedule: Monday to Friday, 9 AM - 5 PM

Setting: Skilled Nursing Facility

Key Responsibilities:
  • Conduct thorough assessments and develop individualized treatment plans tailored to skilled nursing residents
  • Provide physical therapy interventions such as therapeutic exercises, gait training, balance activities, and manual therapy
  • Collaborate with nursing staff, physicians, and other therapists to optimize patient outcomes
  • Document patient progress accurately and timely in compliance with regulatory standards
  • Educate residents and families on rehabilitation goals and home exercise programs
  • Maintain compliance with all state and federal regulations
