POSITION SUMMARYAssesses and restores patient's function, alleviates pain, and prevents disabilities by designing, planning and implementing medically prescribed physical therapy interventions. Provides superior customer service by modeling the Brand Promise and Core Values.
RESPONSIBILITIES- Treatment & Planning. Meet patient goals and needs by providing quality, evidence-based care; assess, develop, and implement an individualized treatment plan; help the patient accomplish the treatment plan and accept therapeutic devices by administering manual exercises; instruct, encourage, and assist patients in performing physical activities, such as non-manual exercises, ambulatory functional activities, and daily-living activities
- Discharge Management. Assure continuation of therapeutic plan following discharge by designing home exercise programs; instruct patients, families and caregivers in home exercise programs; recommend and provide assistive equipment; recommend outpatient or home health follow-up programs
- Leadership. Develop physical therapy staff by providing information; develop and conduct in-service training programs
REQUIRED QUALIFICATIONS- Master's Degree in Physical Therapy or Bachelor's degree with prior experience as a licensed Physical Therapist required.
- Required Licensure/Cerification: Current New York State license or a limited permit as a Registered Physical Therapist
- If a New York State license or limited permit is not held, must be hired within 90 days of graduation from a New York State or APTA-accredited physical therapist education program and meet Practice Exemption under New York State Physical Therapy License Requirements.
- If working in Homecare or traveling between locations, an NYS license is required.

EDUCATION:LICENSES / CERTIFICATIONS: PHYSICAL REQUIREMENTS:M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.
