Physical Therapist (PT) to join our exceptional in-house rehabilitation team in Salinas, California.
As a leading short-term rehabilitation and skilled nursing facility in Monterey County, we provide advanced sub-acute and restorative care to help residents regain mobility, independence, and quality of life. Our facility offers a dynamic clinical environment where therapists are supported, valued, and encouraged to grow professionally.
Key Responsibilities
- Perform comprehensive physical therapy evaluations and develop individualized treatment plans based on resident goals and functional needs.
- Provide skilled physical therapy interventions, including gait training, therapeutic exercise, balance training, manual therapy, and neuromuscular re-education.
- Monitor resident progress and adjust treatment plans to maximize functional outcomes.
- Guide residents through their rehabilitation journey from acute recovery toward safe discharge and community reintegration.
- Collaborate with interdisciplinary team members to coordinate effective, resident-centered care.
- Supervise Physical Therapist Assistants (PTAs) in accordance with California practice guidelines.
- Maintain accurate and timely documentation, including evaluations, daily treatment notes, progress reports, recertifications, and discharge summaries.
- Participate in care conferences, clinical meetings, and quality improvement initiatives.

Qualifications
- Current California Physical Therapist (PT) license in good standing.
- Strong clinical reasoning, communication, and teamwork skills.
- Passion for geriatric, sub-acute, and functional rehabilitation.
