- Pay Rate:$57.00-$65.00+ per hour, depending on experience

Work alongside a multidisciplinary rehabilitation team of
PTs, PTAs, OTs, and SLPs, supported by clinicians with
10+ years of experience. Our team values collaboration, clinical growth, and mutual respect.
Your Role & Impact
As a Physical Therapist, you will play an essential role in helping residents achieve their highest level of function and independence.
Responsibilities include:- Perform comprehensive physical therapy evaluations and develop individualized, evidence-based treatment plans.
- Provide skilled therapy services to short-term rehabilitation and long-term care residents.
- Treat patients recovering from joint replacements, cardiac events, strokes, and other medical conditions.
- Utilize specialized rehabilitation programs, including Acute Neuro Rehabilitation and Pain Management.
- Monitor patient progress and modify treatment plans as appropriate.
- Collaborate with PTAs, OTs, SLPs, nursing, and other members of the interdisciplinary team.
- Educate patients and families regarding treatment plans, exercises, mobility, and strategies for continued progress.
- Promote safe mobility, functional independence, and successful discharge planning.
- Maintain accurate and timely clinical documentation.
- Advocate for patients and support their goals throughout the rehabilitation process.

Qualifications
- Current California Physical Therapist (PT) license required.
- Previous experience in skilled nursing, post-acute, or rehabilitation settings preferred.
- Strong clinical reasoning and communication skills.
- Passion for rehabilitation and helping patients achieve meaningful functional outcomes.
- Commitment to evidence-based, patient-centered care.
- Ability to work collaboratively as part of a multidisciplinary rehabilitation team.
