You'll guide residents through recovery in a skilled nursing setting, building treatment around the movements they need to navigate daily life.
What You'll Do- Evaluate strength, balance, mobility, and functional limitations
- Establish individualized goals and provide physical therapy treatment
- Address gait, transfers, and fall prevention through practical activities
- Reassess outcomes and update plans as residents' abilities change
- Document care and share recommendations with nursing and the rehab team
