Responsibilities

  • Complete comprehensive assessments and OASIS data promptly
  • Assist with orientation and supervision of Physical Therapy Assistants
  • Report patient treatment responses and significant changes to healthcare providers
  • Educate and support patients and their families

What You'll Do
  • Conduct an initial evaluation/assessment and report findings to the physician / provider and Clinical Manager and develop the plan of care promptly for patients
  • Perform physical therapy interventions, procedures, measurements, evaluations and diagnostic tests on patients in the home
  • Document initial evaluation and treatment plan and re-evaluations of patient's progress every thirty (30) days and updates plan of care as necessary
  • Complete comprehensive assessments and OASIS data, as indicated on a timely basis
  • Assist with orientation, supervision and consultation services to Physical Therapy Assistants
  • Observe, record and report to Health Care Provider the patient's treatment, response and progress and promptly report significant changes and obtain orders to revise plan of care
  • Communicate with physician / provider and Health Care Provider and other members of the health care team for the delivery and coordination of quality physical therapy services
  • Provides education and support to patients and their families

  • Strong documentation, time management, and communication skills
  • Experience with Home Care Home Base (HCHB) preferred (training available)
