What You'll Do- Conduct an initial evaluation/assessment and report findings to the physician / provider and Clinical Manager and develop the plan of care promptly for patients
- Perform physical therapy interventions, procedures, measurements, evaluations and diagnostic tests on patients in the home
- Document initial evaluation and treatment plan and re-evaluations of patient's progress every thirty (30) days and updates plan of care as necessary
- Complete comprehensive assessments and OASIS data, as indicated on a timely basis
- Assist with orientation, supervision and consultation services to Physical Therapy Assistants
- Observe, record and report to Health Care Provider the patient's treatment, response and progress and promptly report significant changes and obtain orders to revise plan of care
- Communicate with physician / provider and Health Care Provider and other members of the health care team for the delivery and coordination of quality physical therapy services
- Provides education and support to patients and their families

Qualifications- Doctorate or Master's degree in Physical Therapy from an accredited program
- Active Physical Therapy license in the state of practice
- Minimum 1 year of experience as a Physical Therapist (home health preferred, not required)
- Valid driver's license, reliable transportation, and auto insurance
- Ability to provide physical therapy evaluations and treatments in the home setting
- Strong documentation, time management, and communication skills
- Experience with Home Care Home Base (HCHB) preferred (training available)
