We are seeking an energetic, self-motivated, hardworking, and dependable Physical Therapist (PT) to provide exceptional care to clients in the comfort of their own homes. The ideal candidate is organized, adaptable, and able to multitask effectively while managing a dynamic caseload.

Responsibilities- Complete initial and ongoing comprehensive assessments as assigned.
- Provide interventions and therapeutic treatment
- Effectively communicate with other members of the interdisciplinary healthcare team to promote the coordination of patient care and planning for discharge.
- Participate in Quality Assessment and Performance Improvement (QAPI) program.
- Responsible for reporting on the condition of patient to the Rehab Director, and other appropriate staff members.
Required Qualifications- Current Physical Therapist (PT) licensure in the state of Colorado
- Current Basic Life Support (BLS) CPR or willing to obtain upon hire (demonstration course required)
- One year of PT experience
- Home health care experience preferred, but not required
- Pass a background check and drug test (we do to test for THC)
- No Covid Vaccination Required
