Seeking Physical Therapists in the Home Health settingPer-Visit Rates:$75 per regular PT Visit
$112.50 per PT Eval
$187.50 per PT SOC
Job Description:Physical Therapist is responsible for comprehensive assessments in the patient's home, including the completion of skilled documentation through Homecare Homebase.
- Evaluate patients and perform appropriate therapy interventions
- Develop a comprehensive plan of care and coordinate multi-disciplinary care
- Perform prompt clinical documentation and visit notes
- Continued communication with physician, care team members, patient and family
- Comfortable working in all aspects of the home health setting
- Implement and maintain physical therapy protocols
- Manage patient treatment and outcomes directly, or through PTA supervision
Requirements:- Current Texas Physical Therapist (PT) license
- One year of physical therapy experience
- Experience with Homecare Homebase is helpful, but not required
- Previous Home Health experience is helpful, but not required
- Current Texas driver's license
- CPR certification
