- 5x 8 hour shifts (8:30AM to 4:30PM) - Rotating Weekends

Job Details
Our Home Care PTs:
- Review patient charts and medical history
- Develop a personalized treatment plan
- Teach therapeutic exercises and stretches to improve patient’s conditions
- Use hands-on therapy to promote healing
- Maintain patient records by keeping track of goals and progress while adapting treatment plans accordingly
- Work closely with a supportive patient care team including physicians, nursing, social services, pastoral care, home health aides and others for your assigned caseload.
- Provide patient and family education for health promotion as directed by physician’s plan of treatment.
- Provide care to the most upmost standard
Minimum Qualifications:
- New York State PT license
- Minimum of one year of recent hands on experience as a Physical Therapist.
- Valid New York State Driver’s License and reliable transportation
Posted Salary RangeUSD $95,000.00 - USD $120,000.00 /Yr.
