Estimated Gross Weekly Pay: $3000

$500 CONTRACT COMPLETION BONUS

Opportunity:

Physical Therapist - Physical Therapy

Job Details:
• Monday through Friday schedule from 8:00 AM to 5:30 PM
• 40 guaranteed hours per week
• Primary location is the Medford site with occasional travel to central and northern regions
• Mileage reimbursed at government approved rate
• No on-call or call-back responsibilities
• Physical Therapists rotate one Saturday every 6-8 weeks with a weekday off given in exchange; no holiday rotation required
• Evaluate, treat, and plan patient care in accordance with Physical Therapy standards of practice
• Serve adult and geriatric patient population with a variety of orthopedic, neurological, and functional mobility diagnoses
• Meet productivity expectations of an average of 4.5 visits per day
• Document to meet regulatory and reimbursement requirements using laptop for point of service documentation, NetSmart EMR, and cell phone for GPS and communication
• Scrub colors: any or business casual attire acceptable
• Required certifications: AHA or ARC BLS certification required
• Wisconsin state licensure required; NPI number required (candidates without one must begin application process prior to start)
• Master's Degree in Physical Therapy required
• Minimum experience required: Home Health experience with training in adult and geriatric populations; knowledge of Physical Therapy standards and practices
• Must live outside a 50-mile radius from facility
• Strong work ethic, resilience, adaptability to change, and social/interpersonal skills preferred
