You'll have the opportunity to:
- Build lasting relationships with participants and their families.
- Evaluate functional abilities and develop individualized treatment plans.
- Recommend home modifications and adaptive equipment that improve safety and independence.
- Collaborate daily with physicians, nurses, physical therapists, social workers, and other disciplines.
- Help participants maintain meaningful daily activities and improve their quality of life.
- Play an important role in launching a new PACE program and helping establish best practices.
Instead of seeing patients for just a few visits, you'll become part of a team that follows participants over time, allowing you to truly see the impact of your care.
QualificationsWe're looking for someone who is passionate about helping older adults thrive.
Required:- Graduate of an accredited entry level program in Physical Therapy
- Active Colorado Physical Therapist license
- Current CPR certification
- Valid Colorado driver's license and proof of insurance
- One year working with elderly population, home care or geriatric experience
Preferred:- Ability to work effectively with culturally, economically, and educationally diverse populations
- Experience working with interdisciplinary teams
- An interest in preventative, community-based care
