Salary: $80,308 - $87,900 annually

Pathways PACE is excited to have a brand-new Greeley location, and we're looking for a Physical Therapist who wants to help shape the future of care for older adults in Northern Colorado. This isn't just another PT position.

As a Physical Therapist with Pathways PACE, you'll work as a valued member of an interdisciplinary team that truly collaborates to help participants remain healthy, independent, and living in the community.

You'll have the opportunity to:

• Build lasting relationships with participants and their families.
• Evaluate functional abilities and develop individualized treatment plans.
• Recommend home modifications and adaptive equipment that improve safety and independence.
• Collaborate daily with physicians, nurses, physical therapists, social workers, and other disciplines.
• Help participants maintain meaningful daily activities and improve their quality of life.
• Play an important role in launching a new PACE program and helping establish best practices.

Instead of seeing patients for just a few visits, you'll become part of a team that follows participants over time, allowing you to truly see the impact of your care.
Qualifications

We're looking for someone who is passionate about helping older adults thrive.

Required:

• Graduate of an accredited entry level program in Physical Therapy
• Active Colorado Physical Therapist license
• Current CPR certification
• Valid Colorado driver's license and proof of insurance
• One year working with elderly population, home care or geriatric experience

Preferred:

• Ability to work effectively with culturally, economically, and educationally diverse populations
• Experience working with interdisciplinary teams
• An interest in preventative, community-based care
