$5,000 Sign-On Bonus

Location: Lakeport, CA

Compensation:
  • Full-Time: $70.00 - $75.00 per hour + Full Benefits
  • Travel/PRN: Up to $95.00 per hour
  • Sign-On Bonus: $5,000 (Full-Time)

Essential Responsibilities
  • Evaluate patients within required regulatory timeframes following referral
  • Develop individualized treatment plans based on patient needs and goals
  • Deliver evidence-based physical therapy interventions in accordance with physician orders
  • Monitor patient progress and adjust treatment plans as appropriate
  • Supervise Physical Therapist Assistants (PTAs) and support staff
  • Collaborate with physicians, nursing staff, therapy team members, and families
  • Participate in care conferences, discharge planning, and interdisciplinary meetings
  • Educate patients, families, and caregivers on therapeutic programs and safety measures
  • Maintain accurate and timely documentation in compliance with state, federal, and facility requirements
  • Assist with recommendations and procurement of adaptive and durable medical equipment
  • Ensure therapy equipment is properly maintained and functioning safely

Qualifications
  • Active California Physical Therapist License in good standing
  • Bachelor's Degree in Physical Therapy required
  • Master's or Doctorate in Physical Therapy preferred
  • Skilled Nursing Facility (SNF), Post-Acute Care, Rehabilitation, or Long-Term Care experience preferred
  • Strong clinical assessment and treatment planning skills
  • Excellent communication, organization, and documentation abilities
  • Experience with electronic medical records, including Casamba and PointClickCare, preferred
  • Ability to work effectively within an interdisciplinary healthcare team
