Benefits day 1 Schedule - Monday-Friday-Days Mileage reimbursements Role: The Physical Therapist position is an integral member of the dedicated inter-disciplinary team that provides important short term / long term care of our patients. They provide a high level of care for our patients in the client homes and in facilities. This is a Full Time position working days Monday - Friday.  Key Responsibilities o Provide & direct physical therapy services o Initiate physical therapy program & instruct other personnel &/or family/caregivers in certain phases of the physical therapy with which they may work with the patient o Evaluates & assesses patients o Documents initial evaluations, progress notes, daily records in participant Medical Records, including Plan of Care o Collaborate with other team personnel to achieve well-rounded care  Knowledge & Skills o Excellent communication; via phone, in person and email o Attention to detail & ability to handle confidential information following HIPPA guidelines  Required qualifications o Bachelor's degree required & Master’s program in Physical Therapy preferred, accredited organization o Board Certified Music Therapist (MT-BC) or eligible to become o Minimum 1 year experience working with frail / elderly population o 2 years experience as a Physical Therapist o Community / home health experience preferred o Valid Florida Drivers' License with good record o Automobile Insurance o Possess & maintain current CPR certification o Clear level 2 background check Educational/Training Requirements Bachelors Certificates/Licenses/Registrations Current license as PT in Florida. Must possess a valid Florida driver's license, current auto insurance, and reliable transportation. Certification of completion of Alzheimer's Disease and Related Dementias Training through the Florida Department of Elder Affairs. Experience Requirements One to two years related experience and/or training; or equivalent combination of education and experience. Position Specific Requirements (not all inclusive) Responsible for the initial assessment and ongoing assessment of patients upon referral from multi-disciplinary team. Evaluation of patient/family response to therapy. Responsible for providing and recommending therapeutic functions. Identify and identify problems and develop solutions. Develop goals as related to the needs of the patient. Develop treatment plan in consultation with the IDG to include the physician, including any changes or provisions. Revise plan of care as needed to include discharge planning and evaluation. Review and complete patient assessments and plan of care. Identify equipment required to increase the patient's function and independence. Plan and develop maintenance, exercise and therapy programs for participants. Provide education to family members and friends for those parts of the treatment plan that they can carry out. Responsible for timely documentation of all therapy interventions to include assessments and evaluation data, treatments and the patient's response to therapy intervention. Oversee duties of Physical Therapy Assistant and PT/OT Aide. Represent the company in a positive and professional manner at all times with others, e.g., co-workers, management, vendors, and customers. Consistently represents Hope with integrity and professionalism. Complete all job responsibilities in a thorough and complete manner, utilizing good judgment in all aspects of the job. Carry out duties and responsibilities in a manner that exemplifies excellent customer service. When unsure of a directed task or any part of job responsibilities, proactively seek the assistance of immediate supervisor. Assist with special projects, as requested. All other job duties as may be assigned. Compensation Pay Range: $76,133.58 - $118,958.71
