Job Title: Physical Therapist

Experience: Five years of progressively responsible Physical Therapy experience; minimum of two years in a clinical setting with a frail or elderly population.
License: Valid Physical Therapy License
Certifications: BLS
Must-Have:
- Physical Therapy knowledge and skills necessary to treat frail, elderly participants with complex needs.
- Highly motivated and self-directed, with the ability to make sound decisions in emergency situations.
- Two years of supervisory experience with a demonstrated ability to mentor and develop team members.
- Strong leadership, organizational, and communication skills in settings with seniors and interdisciplinary team members.
- Ability to work independently with minimal supervision.
- Commitment to unlocking the full potential of vulnerable seniors.
Description:
The Physical Therapist is responsible for coordinating and providing restorative and rehabilitative Physical Therapy services to participants.
This role involves working in close coordination with the Physician, rehabilitation staff, and other interdisciplinary team members to maximize participant independence and safety.
The Physical Therapist will conduct assessments to determine care needs, preferences, and goals of the participants.
Active participation in interdisciplinary team meetings to develop participant care plans is required.
Physical Therapy interventions will be delivered and documented in accordance with physician orders and agreed-upon care plans.
These interventions include active and passive exercise, muscle re-education, gait training, and functional training of activities of daily living.
Supervision of Therapy Assistants will include training, instruction, and guidance for implementing Functional Maintenance Programs.
Training and instruction will also be provided to staff, participants, and caregivers in the use and care of therapeutic equipment.
The Physical Therapist will remain alert to input from team members, participants, and caregivers, updating the team promptly about any changes in participants' conditions or functional statuses.
Collaboration with interdisciplinary team members will support smooth care transitions between various settings.
This role includes participation in end-of-life care coordination and support.
The Physical Therapist will manage direct reports, including training and mentoring.
Legally authorized practice in the position location is required.
The individual must complete all required documentation timely and accurately.
Privacy and confidentiality of all procedures and information about team members and families must be maintained.
Participation in quality improvement activities and adherence to policies and procedures are necessary.
Regular communication with the supervisor and team regarding workload and priorities is expected.
Flexibility in performing assignments is important as business needs evolve.
Other duties may be assigned as needed.
