Description:
The Physical Therapist is responsible for coordinating and providing restorative and rehabilitative physical therapy services to participants in the center and at home.
The therapist works in close coordination with physicians, rehabilitation staff, and other interdisciplinary team members to maximize participant independence and safety and to develop or restore function.
The Physical Therapist collaborates with the Occupational Therapist to efficiently operate the rehabilitation department.
The therapist is a member of the management team and facilitates collaborative relationships across the organization to promote values, team culture, and mission.
Key duties include conducting physical therapy assessments to determine care needs, preferences, and goals of participants.
This role involves delivering and documenting physical therapy interventions in accordance with physician orders and care plans.
The therapist provides active and passive exercise, muscle re-education, gait, and functional training of activities of daily living.
Supervision of therapy assistants includes providing training, instruction, and guidance needed to implement functional maintenance programs.
The Physical Therapist provides training and instructions to staff, participants, and caregivers in the use and care of therapeutic equipment.
This role includes ordering, tracking, and managing maintenance for equipment.
The Physical Therapist remains alert to pertinent input from team members, participants, and caregivers.
They update the interdisciplinary team promptly regarding any changes in participant conditions or functional status.
Collaboration with team members supports smooth care transitions between settings such as hospitals, skilled nursing facilities, and homes.
Participation in end-of-life care coordination and support is also expected.
The Physical Therapist manages direct reports, including training, supervising, and mentoring.
They must complete all required documentation accurately and in a timely manner while protecting privacy.
This role requires adherence to organizational policies and participation in mandated training and education.
Regular communication with supervisors and teams about workload and priorities is essential.
Flexibility in performing assignments as business needs evolve is required.
Other duties may be assigned as necessary.
