Physical Therapist (PT) - Home Health
Schedule: Full-time Monday-Friday
Location: Field Based (Houston Pod)
Compensation (Productivity Points): $75 per point
What you will do:
Own the caseload as a case manager: you're accountable for the patient's therapy plan, progression, and outcomes - not just individual visits.
Complete comprehensive in-home assessments and deliver skilled PT visits.
Build and execute a clear plan of care: gait/balance, transfers, strengthening, functional mobility, home safety, fall prevention.
Set measurable goals, track objective progress, and adjust treatment quickly when patients stall.
Drive multidisciplinary coordination: communicate changes in condition, align on orders, and keep the care team moving.
Manage treatment and outcomes directly, including PTA supervision where appropriate.
Coach patients and caregivers with practical home programs that actually get done.

Active Texas PT license (in good standing)

Current driver's license and comfort with daily travel across assigned territory
