Home Health Physical Therapist (Hybrid Opportunity)

Territory: Central Kitsap Bremerton/Silverdale Region
Type: Full-Time
Pay: $45.00 – $58.00 per hour
Schedule: Monday – Friday, 8:00 AM – 5:00 PM

Position Overview

The Home Health Physical Therapist provides rehabilitative and skilled physical therapy services to patients primarily in their place of residence. This role also includes a hybrid outpatient component, offering opportunities to deliver therapy services in alternative clinical or community-based settings as organizational and patient needs evolve. The hybrid structure allows flexibility while maintaining a strong focus on patient-centered care and functional outcomes.


Duties and Responsibilities
  • Provides rehabilitative and skilled physical therapy services to patients in the home and, when applicable, in outpatient or community-based care settings
  • Assumes responsibility for ongoing interdisciplinary assessment and development of individualized plans of care in partnership with patients, representatives, and caregivers
  • Delivers patient, caregiver, and family education to support independence, safety, and continuity of care
  • Assesses and evaluates functional status, including strength, mobility, gait, balance, ROM, and overall rehabilitation potential
  • Develops and directs physical therapy treatment programs aligned with physician orders and patient goals
  • Evaluates environmental and functional needs to support safe participation in daily activities across care settings
  • Prepares and maintains accurate clinical documentation, including evaluations, progress notes, and discharge summaries
  • Communicates and coordinates with physicians and other healthcare professionals regarding patient progress and plan-of-care updates
  • Instructs caregivers and other personnel in components of the therapy program as appropriate
  • Identifies needs for additional services and makes referrals to other disciplines as necessary
  • Participates in care conferences, discharge planning, and peer consultation activities
  • May provide oversight and supervision of Physical Therapy Assistants in accordance with organization policy and state regulations
Qualifications / Skillsets
  • Licensed to practice as a Physical Therapist in the state of Washington
  • Graduate of a CAPTE-accredited Physical Therapy program (Bachelor’s or Master’s degree)
  • Minimum of two (2) years of experience as a Physical Therapist; home health and/or outpatient experience preferred
  • Strong verbal, written, and organizational communication skills
  • Current CPR certification
  • Ability to travel within assigned service areas; must maintain a valid driver’s license, auto insurance, and reliable transportation
