Join Our Team as a Hospice and Palliative Care Nurse PractitionerDo you value the time you spend with patients? Are you committed to providing quality, compassionate care? We are looking for nurse practitioners who are committed to creating meaningful patient experiences.

 

As a hospice and palliative care nurse practitioner, you’ll provide patients with diagnosis, treatment, and care under the supervision of a physician. In this role, you’ll focus on providing palliative care. Most importantly, you’ll assist families along their health care journeys with love and compassion.

Qualifications
  • A heart to serve patients and families and a passion for providing the best possible care
  • Education: Graduate of an accredited masters, post masters, or doctoral nursing program with formal preparation as a Nurse Practitioner.
  • Licensure: Current unencumbered licensure as an Advanced Practice Registered Nurse with designation as a Nurse Practitioner. Current prescriptive authority with full DEA privileges. Must achieve and maintain national certification in the Nurse Practitioner specialty
  • Experience: 2 years experience within the last 5 years in chronic disease management/long-term care. Hospice and/or Palliative background preferred.
  • Required: Reliable transportation. Ability to sit, stand, bend, move intermittently and lift at least 80-100 lbs.
