Position OverviewHome Health Physical Therapist Elite Home Health & Hospice is seeking a
full-time Physical Therapist to join our high-performing home-health and hospice team in
Lewiston, Idaho. This is a relocation-friendly opportunity for PTs who want meaningful patient relationships, clinical autonomy, and the chance to work with one of the most respected home-health organizations in the region.
Key ResponsibilitiesPT Responsibilities- Perform comprehensive physical therapy evaluations in patient homes.
- Develop individualized treatment plans focused on functional mobility, safety, and independence.
- Provide therapeutic interventions including gait training, strengthening, balance work, and home-exercise program development.
- Conduct home-safety assessments and recommend appropriate DME or environmental modifications.
- Manage a field-based caseload with strong organizational efficiency.
- Collaborate with interdisciplinary team members to ensure coordinated, patient-centered care.
- Maintain accurate, timely documentation in accordance with regulatory and organizational standards.
- Support safe transitions from hospitals, SNFs, and outpatient settings.
ScheduleFull-Time Schedule- Monday-Friday
- Daytime schedule

QualificationsPT Requirements- Current Idaho Physical Therapist license (or ability to obtain).
- One year of PT experience preferred; home-health experience a plus.
- Strong communication, assessment, and clinical reasoning skills.
- Ability to work independently in a field-based environment.
- Reliable transportation and valid driver's license.
Compensation & BenefitsCompensation Details- Competitive salary: $105,000-$135,000
