Position Overview

Home Health Physical Therapist Elite Home Health & Hospice is seeking a full-time Physical Therapist to join our high-performing home-health and hospice team in Lewiston, Idaho. This is a relocation-friendly opportunity for PTs who want meaningful patient relationships, clinical autonomy, and the chance to work with one of the most respected home-health organizations in the region.

Key Responsibilities

PT Responsibilities
  • Perform comprehensive physical therapy evaluations in patient homes.
  • Develop individualized treatment plans focused on functional mobility, safety, and independence.
  • Provide therapeutic interventions including gait training, strengthening, balance work, and home-exercise program development.
  • Conduct home-safety assessments and recommend appropriate DME or environmental modifications.
  • Manage a field-based caseload with strong organizational efficiency.
  • Collaborate with interdisciplinary team members to ensure coordinated, patient-centered care.
  • Maintain accurate, timely documentation in accordance with regulatory and organizational standards.
  • Support safe transitions from hospitals, SNFs, and outpatient settings.


Schedule

Full-Time Schedule
  • Monday-Friday
  • Daytime schedule
  • Balanced caseload with strong team support
