We are currently seeking a motivated and experienced full-time Physical Therapist to join our team at a partnered Assisted Living Facility on the Upper East Side (NYC). All levels of experience are encouraged to apply and will train the right new graduate.

Job Type: Full-Time w/ benefits
Pay: 90k-105k
Location: NYC - Upper East Side, 80th st. (Assisted Living Facility)
Schedule: Super flexible!

  • Provide 1:1 physical therapy treatment to residents within the facility
  • Evaluate patients and develop individualized treatment plans based on physician orders and patient needs
  • Deliver care in accordance with professional standards and the established plan of care
  • Lead a weekly group exercise class focused on mobility and functional wellness
  • Collaborate with facility staff and caregivers to support patient outcomes
  • Plan and implement therapy programs to improve or restore functional abilities, reduce limitations, and enhance quality of life
  • Perform and document initial evaluations, progress notes, and discharge summaries in a timely and accurate manner
  • Monitor patient progress and modify treatment plans as needed to achieve optimal results
  • Provide care with a strong understanding of geriatric and memory care populations


Qualifications
  • Physical Therapist Program Graduate
  • Licensed Physical Therapist in the state of New York or eligible.


Additional Information

The anticipated base salary range for this position is $90,000.00-$105,000.00. Salary is based on various factors, including relevant experience, knowledge, skills, other job-related qualifications, and geography. Medical, dental, vision, 401(k), paid time off, and other benefits are also available, subject to the terms of the Company's plan.
