Physical Therapist Position Highlights: - Setting: Onsite Required ( No Remote, Telehealth, or Hybrid) - 5 Star Medicare Skilled Nursing Facility
- Competitive Pay: $60-$65 per hour(approx.$124,800 - $135,200 Annual Salary)

- Patient Type: Adult and Geriatric Patients
- EMR System: Net Health
- Flexible Schedule: Monday - Friday, Sunday - Thursday, or Tuesday - Saturday
- Shift/ Hours: 8:00 am -5:00 pm - 5x 8s - 40 hours per week
Responsibilities: - Conduct comprehensive wound assessments and develop individualized treatment plans in collaboration with physicians and interdisciplinary teams.
- Perform Wound Mist Therapy treatments in compliance with state regulations and facility protocols.
- Implement evidence-based wound care interventions, including offloading, compression therapy, debridement recommendations, and infection control measures.
- Maintain accurate and timely documentation in accordance with Medicare, Medicaid, and CA state healthcare regulations.
- Educate patients, caregivers, and facility staff on wound care best practices and prevention strategies.
- Collaborate with physicians, nurses, dietitians, and other allied health professionals to ensure comprehensive patient care.
- Monitor treatment efficacy and wound healing progress, making necessary adjustments to treatment plans.
- Adhere to HIPAA guidelines and maintain patient confidentiality.
- Stay up to date with current wound care advancements and participate in ongoing professional development.
Qualifications & Requirements: - 1 or more years of Physical Therapy experience preferred
- New Grad Friendly - Mentorship Available - Active CA PT License Required
- Active California Physical Therapist (PT) License Required - Must be in good standing with no disciplinary actions
- Active BLS Card Required - American Heart Association or American Red Cross
- Minimum of 1 year of experience in wound care, rehabilitation, or skilled nursing facility (preferred but not required).
- Certification in wound care (e.g., WCC, CWS, CWPT) is a plus.
- Knowledge of Medicare/Medicaid guidelines for wound care services in skilled nursing facilities.
- Ability to work independently and within an interdisciplinary team.
- Strong clinical reasoning, patient education, and documentation skills.
- Familiarity with electronic medical records (EMR) systems.
