Coverage Area: Palatka, FL 32178 / Putnam County
(Territories designed to minimize drive time with clustered scheduling)
Compensation & Earning Potential- Pay-per-visit model: $70+ per visit + Medicare differential pay
- Average full-time earnings: $88,000 *SALARY* for 4 days a week with additional earning opportunities
- Quarterly bonus opportunities
- Mileage reimbursement + data plan stipend
Consistent patient volume provided
Your Role as a Home Health Physical Therapist- Evaluate patients and develop individualized treatment plans
- Provide therapeutic exercises and mobility training
- Educate patients and caregivers on safety and recovery
- Collaborate with physicians and interdisciplinary teams
- Supervise Physical Therapist Assistants (PTAs)
- Complete documentation using streamlined technology
