Essential Duties & Responsibilities
• Provide comprehensive medication management, including medication histories, reconciliation, adherence assessments, and patient education.
• Deliver evidence based, patient centered drug therapy recommendations and collaborate with the interdisciplinary care team.
• Evaluate and verify medication orders for accuracy, appropriateness, dosing, interactions, contraindications, and duplicate therapy.
• Identify, prevent, and resolve medication related problems; support patient safety, quality improvement, and clinical guideline initiatives.
• Serve as the primary clinical pharmacy resource for patients, providers, and staff.
• Apply 340B program knowledge to ensure compliant, affordable medication access and sound pharmacy operations.

• PharmD or equivalent doctoral degree - Required
• 3-5 years outpatient or retail pharmacy experience - Preferred
• Active Arizona Pharmacist License - Required upon hire
• BLS certification including in person skills component - Required upon hire
