Full Job Description
We are seeking qualified Pharmacist to service our patients though out the Charleston area. Pharmacists play a vital role in ensuring the safe and effective use of medications to improve patient outcomes. You will collaborate with interdisciplinary care teams to optimize medication therapy and promote medication adherence. Your responsibilities include medication management, patient consultations, care coordination, and care management, all aimed at enhancing patient care and reducing rehospitalizations.

Our pharmacist conducts virtual patient visits from the comfort of their own homes.This is a full-time salary-based position, with the opportunity to receive a monthly bonus.

The following shift(s) are available:
  • 8-hour shift (Monday-Friday; 8:00am-5:00pm)

Your Health is currently looking for a Pharmacist to join our growing primary care family. A successful Pharmacist will be able to perform these essential duties and responsibilities. Areas of Responsibility:
  • Ensure the accurate dispensing, compounding, and distribution of medications in compliance with state and federal regulations.
  • Verify prescription orders, assess medication appropriateness, and counsel patients on proper medication use, dosage, and potential side effects.
  • Monitor medication therapy outcomes, adverse drug reactions, drug interactions, and make recommendations for therapy adjustments as needed.
  • Provide medication therapy management (MTM) services, including comprehensive medication reviews, medication reconciliation, and adherence assessments.
  • Conduct patient consultations via telehealth or in-person visits to address medication-related concerns, optimize treatment plans, and promote medication adherence and lifestyle modifications.
  • Collaborate with interdisciplinary care teams to develop and implement comprehensive care plans for high-risk patients with complex medical needs.
  • Participate in Care Group/Care Team meetings as necessary.
  • Evaluate patients' medication regimens, identify potential gaps in care, and coordinate medication adjustments to optimize therapeutic outcomes and prevent rehospitalizations.
  • Communicate effectively with case managers, clinical coordinators, social workers, and other care team members to ensure coordinated care delivery and seamless transitions of care.
  • Collaborate with internal and external resources to facilitate and ensure seamless operations.
  • Meet staff efficiency expectations by performing consultations, medication reviews, and other clinical pharmacy services efficiently and effectively.
  • Identify opportunities to reduce medication costs, improve formulary adherence, and optimize medication utilization through evidence-based prescribing practices and medication management strategies.
  • Maintain accurate and up-to-date documentation of patient encounters, medication therapy interventions, and clinical recommendations in electronic health records (EHR) systems.
  • Generate reports, analyze medication-related data, and participate in quality improvement initiatives to enhance patient care outcomes and medication safety.

Qualifications:
