ABOUT THE POSITION (Full-Time, On site)This Onsite Pharmacist is an integral member of the healthcare team in Colorado, providing pharmacy and clinic-based support across specialty and ambulatory oncology settings. This role partners directly with the oncology providers and patients to manage medication therapy, deliver patient education, and ensure safe, effective, and continuous care throughout the treatment journey.
This position blends clinical practice, patient-facing education, interdisciplinary collaboration, and quality-improvement leadership. The Pharmacist contributes to medication-safety initiatives, operational efficiency, and institutional goals while maintaining a strong focus on patient-centered care, clinical excellence, and evidence-based practice
Principal Duties and Responsibilities - Oversee medication therapy management in alignment with organizational policies and protocols.
- Identify, report, and resolve medication errors, adverse drug reactions, and safety issues.
- Respond flexibly to changing departmental needs and tasks assigned by leadership.
- Complete timely and accurate documentation in accordance with client and organizational policies.
- Contribute to Quality Improvement (QI) initiatives and maintain records of QI activities.
- Ensure compliance with clinical standards, client policies, and Visante procedures.
- Provide patient-centered medication therapy and address concerns with confidentiality.
- Collaborate with interdisciplinary teams to enhance patient care and program development
- Support initiatives aimed at improving medication safety, operational efficiency, and cost-effectiveness
- Optimize medication use by identifying and resolving drug-related problems
Requirements
Education Required: Bachelor's degree in Pharmacy or Pharm.D. from an ACPE certified school of pharmacy
Preferred: Successful completion of an ASHP accredited PGY1 or specialty residency
Experience Required: 3 years of retail or specialty pharmacy
Licensure/Certification: Required: Current Pharmacist license as granted by the appropriate state licensing authority obtained within 1 months of hire date American Heart Association BLS certification obtained within 6 months of hire date
Preferred: Board of Pharmacy Specialties certification (BCOP, BCACP, BCPS, CSP, etc.)
Special Skills:- Strong understanding of pathophysiology and pharmacotherapeutics
- Ability to interpret and apply regulatory standards
- Demonstration of good judgement, multi-tasking, and meeting deadlines with a sense of urgency and being able to prioritize competing demands
- Strong client relationship, interpersonal, and team skills
- Excellent presentation skills with the ability to make effective presentations to all levels of management
- Proven ability to diagnose and resolve issues, demonstrating strong analytical and creative skills
- Ability to make sound, timely decisions based on analysis, experience, and judgement
- Clear and concise verbal and written communication skills and the ability to advise clients professionally and positively
Compensation:- Good faith salary range: $124,000 to $173,900 annually (based on experience and qualifications). Eligible for performance-based bonus.
