The Hospital at Home Pharmacy Supervisor provides day-to-day leadership and operational oversight for pharmacy services supporting Advocate Health's Hospital at Home program. This role is responsible for ensuring safe, efficient, and compliant medication distribution processes while supporting teammate engagement, quality improvement, regulatory compliance, and service excellence. The Supervisor works closely with pharmacy leadership, nursing, providers, and operational partners to ensure timely medication access for patients receiving acute-level care in the home setting.

In addition to Hospital at Home operations, this position supports pharmacy services across Long-Term Care (LTC), Hospice, Free-Standing Emergency Departments (FSEDs), and MedCenter Air, helping ensure consistent operational performance, regulatory compliance, and medication access across multiple care settings

Pay Range:
$63.45 - $95.20

This role is responsible for ensuring safe, efficient, and compliant medication distribution processes while supporting teammate engagement, quality improvement, regulatory compliance, and service excellence. The Supervisor works closely with pharmacy leadership, nursing, providers, and operational partners to ensure timely medication access for patients receiving acute-level care in the home setting.

In addition to Hospital at Home operations, this position supports pharmacy services across Long-Term Care (LTC), Hospice, Free-Standing Emergency Departments (FSEDs), and MedCenter Air, helping ensure consistent operational performance, regulatory compliance, and medication access across multiple care settings.

Key Responsibilities
• Supervise daily Hospital at Home pharmacy operations and workflow.
• Provide operational leadership and support for LTC, Hospice, FSED, and MedCenter Air pharmacy services.
• Lead staffing, scheduling, and teammate development to support safe patient care.
• Ensure compliance with state and federal regulations, accreditation standards, and pharmacy policies.
• Monitor productivity, service levels, and operational performance metrics.
• Support quality improvement initiatives and implementation of new technologies and services.
• Collaborate with interdisciplinary teams to optimize medication management and patient outcomes.
• Assist with policy development, process improvement, and regulatory readiness activities.
• Participate in leadership meetings, committees, and strategic planning initiatives.

Essential Functions

  • Makes decisions regarding employment, promotion, discipline, and termination of pharmacy team members in collaboration with Pharmacy Leadership.
  • Evaluates pharmacy teammate job performances on an annual basis. Ensures that the pharmacy department adheres to all regulatory bodies including, but not limited to the DEA, the FDA and applicable state Board of Pharmacy.
  • Oversees pharmacy staffing and scheduling to ensure safe patient care.
  • Coordinates and integrates interdepartmental and intradepartmental services and participates in various committees.
  • Ensures that policies and procedures promoting safe and effective use of medications are followed.
  • Assists in the planning of new and/or revised pharmacy services based on new innovations, technology and/or regulatory changes.
  • Assesses and improves the performance of the pharmacy services provided. Provides an ongoing assessment of care and services provided through organizational processes.
  • Coordinates the implementation plan and supports decisions made by the Pharmacy and Therapeutics Committee.
  • Initiates the proper training of pharmacy teammates in area of responsibilities.
  • Designs, recommends, monitors and evaluates patient specific pharmacotherapeutic regimens
  • Supervises daily preparation and dispensing of all pharmaceuticals.


Physical Requirements
Works primarily in the pharmacy. Delivers medications to nursing units. Requires considerable walking, standing, pushing mobile cart of 50 pounds maximum and lifting 25 pounds of material. May be required to frequently stretch, bend, twist, squat, or kneel to perform job. Exposure to cancer chemotherapy agents. Otherwise, hazards are very minor such as minor cuts, bruises, and non-contaminated needle pricks. Exposed to various conditions throughout the workday such as walking on hard surfaces, climbing stairs and changes in temperature and humidity. Must be able to perform manipulative skills such as writing, typing, and data entry into the computer and automated dispensing machines. Must have use of both hands, be able to see clearly and read small type such as that on unit-dose medication packages and syringes. Must be able to speak and be understood by others. Must be able to hear normal sounds with moderate background noises.

Education, Experience and Certifications

Bachelor's Degree in Pharmacy or a PharmD from an accredited school of pharmacy required. Licensed in the state of applicable state or eligible for reciprocity required. 2 years of work related experience in pharmacy required.• Strong operational, regulatory, and team leadership skills.

Prior experience in acute care, ambulatory care, home-based care, LTC, Hospice, or medication distribution preferred. Experience with virtual hospital, logistics, automation, EPIC, Omnicell, and clinical pharmacy

  • Base compensation listed within the listed pay range based on factors such as qualifications, skills, relevant experience, and/or training
