Job Description

Responsible for performing and documenting patient pharmacy clinical interventions, education, and counseling that pertains to medication management and safety. Interprets, evaluates and reviews of physician inpatient orders and outpatient prescriptions in order to prepare, calculate, compound, admix and dispense appropriate medication and dosages consistent with optimum patient care. The interpretation, evaluation and review must include but is not limited to patient drug allergy review, drug-drug, drug-food interaction determination, assessment of therapeutic duplication, over/under dosing and inappropriate therapy and the prompt resolution and documentation of deficiencies found. (I.E., pharmacist is responsible to ensure the dispensation of the right medication, at the right dosage, for the right medical indication, at the right time, to the right patient. Engages in practices that promote and advance professional practice, education, patient safety, and collaboration with other health care practitioners.

Qualifications

Work requires the knowledge of theories, principles, and concepts acquired through completion of a PharmD (BS Pharmacy degree may be accepted as entry level for pharmacist position). NJ Board of Pharmacy Licensure and registration required. Pharmacy Residency and Board Certification in clinical pharmacy practice (e.g. Board Certification in Pharmacotherapy BCPS) Certificate is preferred. New hires have one year from date of hire to receive Advanced Cardiac Life Support Certification (ACLS). Work requires providing direct patient care to neonate, pediatric, adolescent, young adult, adult and geriatric patients. Work requires 25 Continuing Education Units (CEUs) annually as approved by Pharmacy Management.
