Job SummaryServes patients by preparing medications; giving pharmacological information to multidisciplinary health care team; monitoring patient drug therapies.
Performance: Position Specific Essential Functions- Dispenses medications and IV medications for patients ranging in age from neonatal through elderly adults in accordance with local, state, and federal laws and hospital policies and procedures.
- Interprets medication orders and transcribes into the computerized patient medication record accurately.
- Ensures safe, appropriate, cost-effective medication therapies for patients according to established policies, procedures, and protocols
- Participates in the orientation of all new employees; provides in-services education to pharmacy and hospital staff; assists in preparing performance evaluations as required.
- Assures accurate and complete labeling of drugs distributed from pharmacy; maintains records of transactions to assure control and accountability for all drugs.
- Participates in quality improvement and medication use reviews in accordance with department and medical staff approved criteria.
- Sustains the formulary by minimizing non-formulary procurement, utilizing therapeutic substitution protocols, and promoting rational drug therapy selection.
- Provides clinical consultations and clarification to practitioners; suggests appropriate, cost-effective therapeutic alternatives to medical staff, as needed.
- Documents all clinical activities and interventions accurately and completely.
- Performs other duties as assigned.
QualificationsEducation
- Required
- High School Diploma, GED, or equivalent
- Bachelors Degree in Pharmaceutical Science or related
- Doctorate degree in Pharmacy
Experience:
- Preferred
- Two years of experience in pharmaceuticals
Certification/Licensure:
- Required
- Registered Pharmacist by the Texas State Board of Pharmacy
