Full Job Description
Job Summary

Serves patients by preparing medications; giving pharmacological information to multidisciplinary health care team; monitoring patient drug therapies.

Performance: Position Specific Essential Functions
  • Dispenses medications and IV medications for patients ranging in age from neonatal through elderly adults in accordance with local, state, and federal laws and hospital policies and procedures.
  • Interprets medication orders and transcribes into the computerized patient medication record accurately.
  • Ensures safe, appropriate, cost-effective medication therapies for patients according to established policies, procedures, and protocols
  • Participates in the orientation of all new employees; provides in-services education to pharmacy and hospital staff; assists in preparing performance evaluations as required.
  • Assures accurate and complete labeling of drugs distributed from pharmacy; maintains records of transactions to assure control and accountability for all drugs.
  • Participates in quality improvement and medication use reviews in accordance with department and medical staff approved criteria.
  • Sustains the formulary by minimizing non-formulary procurement, utilizing therapeutic substitution protocols, and promoting rational drug therapy selection.
  • Provides clinical consultations and clarification to practitioners; suggests appropriate, cost-effective therapeutic alternatives to medical staff, as needed.
  • Documents all clinical activities and interventions accurately and completely.
  • Performs other duties as assigned.

Qualifications

Education
  • Required
    • High School Diploma, GED, or equivalent
    • Bachelors Degree in Pharmaceutical Science or related
    • Doctorate degree in Pharmacy

Experience:
  • Preferred
    • Two years of experience in pharmaceuticals

Certification/Licensure:
  • Required
    • Registered Pharmacist by the Texas State Board of Pharmacy
