What You'll Do

Position Summary

The Population Health Pharmacist (PHP) at Suvida Healthcare will be a valued team member of the Suvida's Quality and Pharmacy Team, partnering with our center and clinical teams to improve outcomes and experiences for Hispanic older adults and their families. This position sits at the intersection of medications, primary care, and HEDIS/Stars Program - working directly with the quality and pharmacy teams, center primary care teams, and Suvida patients across the enterprise.

By leveraging payer and pharmacy data through our technology platforms, our PHP will apply their clinical acumen to proactively identify medication care gaps, optimize therapeutic regimens, and support various quality initiatives around medications (e.g. adherence, high-risk meds, high-cost meds, etc.). This role is ideal for a pharmacist who is passionate about serving older adults and an analytical mindset that can prioritize objectives to help organizations reach 5-Stars.

Essential responsibilities consist of but not all inclusive:

Responsibilities
• Develop, implement, and continuously improve initiatives focused on medication optimization and adherence and high-risk medications
• Use payer, pharmacy, and EMR data to identify gaps in care and conduct interventions with patients and loop in providers as necessary
• Lead or support quality improvement projects that impact medication-related outcomes and star ratings-related measures
• Become the HEDIS medication measures expert, contributing to individualized care plans for patients at high-risk of failing medication gaps
• Design and deliver education programs around medication measures, both internally to Suvida care teams, as well as externally with a focus on Spanish-speaking older adults
• Provide mentorship and guidance to care team members on medication best practices, and contribute to quality & pharmacy protocols and training content
• Provide patient care services through provider-delegated collaborative practice agreements (e.g. refill approvals, therapeutic interchange)
• Maintain comprehensive knowledge base on all heath plan pharmacy benefits
• Ensure your team, peers, and patients always a positive experience by providing a high level of service promptly, professionally, and empathetically
• Demonstrated ability to work independently, solve problems, and make sound decisions in a timely fashion
• Possess both verbal and written communication skills, interpersonal relationship skills and problem-solving skills

Knowledge, Skills, and Abilities

• Minimum 1-3 years' experience in ambulatory care, primary care, managed care, or population health roles
• Demonstrated ability to interpret clinical and claims data, quality metrics, and value-based performance reports to drive action
• Advanced in Microsoft office platforms including word, excel, and power point
• Requires up to 25% travel to care centers to engage with staff and observe quality workflows in practice.

Education, Experience, Licensure, or Certification Requirements
• Doctor of Pharmacy (PharmD) from an accredited institution
• Active, unrestricted Pharmacy license in Texas, or the ability to obtain one
