We are seeking a passionate and motivated
Pharmacist to join our team full-time at our
Brampton, Ontario location.
At LMC Pharmacy, pharmacists practice in a unique
consultation-based environment focused on diabetes care, education, and chronic disease management. Located within
LMC Healthcare clinics, our pharmacists work collaboratively with endocrinologists, diabetes educators, and allied healthcare professionals to deliver exceptional patient-centered care.
What You'll Do:- Reporting to the Pharmacy Manager, the Pharmacist will support the pharmacy by providing diabetes counseling, training, education, comprehensive medication assessments, and medication therapy management services for patients
- Provide personalized education on diabetes management, medication use, lifestyle strategies, and adherence.
- Build rapport and foster trusted, ongoing patient relationships over the phone and in-person.
- Collaborate with prescribers and other healthcare providers to ensure coordinated care.
- Document consultations and follow-ups in compliance with standards of practice.
- Stay current with diabetes guidelines and best practices to deliver evidence-based care.
- Support all responsibilities related to pharmacy operations including but not limited to medication/supplies dispensing and administration
What We're Looking For:- Interest in helping people with diabetes improve their health and wellbeing
- OCP Part A licensed pharmacist in good standing with current OCP Injection Certification
- 2-3 years of Ontario pharmacy practice and diabetes education experience preferred
- Certified Diabetes Educator (CDE) certification preferred
- Must have previous retail pharmacy experience
- Kroll proficiency and EMR experience an asset
- Excellent verbal and written communication skills
- Excellent interpersonal, organizational and time management skills
- Ability to travel locally to other LMC sites
- South Asian language competency (Punjabi, Hindi) preferred
Compensation range: The expected compensation for the position is $80K-$92K/annually. Actual compensation will be based on a candidate's credentials, experience and geographic location in which the position is located.
