Description

The Pharmacist is an integral member of the interdisciplinary healthcare team, providing specialized clinical pharmacy services to optimize medication therapy and improve patient outcomes primarily within the Emergency Department, with rotation to the central Pharmacy department.

The incumbent collaborates with physicians, nurses, and other healthcare professionals to promote safe, timely, and evidence-based medication use through therapeutic consultation, medication reconciliation, order review, participation in emergency response, and stewardship initiatives. The role supports quality patient care by enhancing medication safety, reducing medication-related risks, and contributing to continuous quality improvement and patient safety initiatives.

Qualifications

Experience

  • Previous experience in an acute care hospital and/or participation in a hospital pharmacy residency program is an asset

Registration/licensure

  • Must be currently registered and in good standing with the Ontario College of Pharmacists (OCP)

Education

  • Bachelor of Science degree in Pharmacy (B.Sc. Pharm) or Doctor of Pharmacy Degree (PharmD)

Knowledge/Skills/Abilities

  • Understanding the roles of various health care professionals and organizations
  • Demonstrated leadership, communication, and interpersonal skills
  • Ability to interact effectively with all members of the healthcare team
