Skills/Experience Required:

Job Summary: Responsible for providing comprehensive, patient-centered, clinical pharmacy services for assigned areas. The Clinical Pharmacist proactively influences medication use and makes recommendations concerning the patient's medication profile. The clinical pharmacist will assess medication orders for the appropriateness of therapy, taking into account patient-specific factors, and evaluate medication regimens to ensure positive outcomes, including relevant laboratory results, tests, and/or procedures. The Clinical Pharmacist will oversee drug distribution and preparation, assuring safety and timeliness. The Clinical Pharmacist will initiate and participate in the collaborative management of patients with other healthcare team members, including medication reconciliation, adherence to core measures, anticoagulation management, targeted drug programs, medication usage guidelines, patient counseling, discharge counseling, and performance improvement initiatives. The clinical pharmacist will participate in continuous readiness efforts to maintain Joint Commission Patient Safety Goals and Medication Management Standards. The Clinical Pharmacist will precept pharmacy and technician students in collaboration with the pharmacy management team. The Clinical Pharmacist will actively participate in developing and presenting educational topics and staff development programs when requested. The Clinical Pharmacist will serve as a primary resource for drug information to patients, medical staff, nursing staff, and personnel from ancillary departments.

Requirements:

- Doctorate of Pharmacy degree or Bachelor of Science degree or equivalent.

- Graduated from an accredited School of Pharmacy.

- Current (ACTIVE) Pharmacist Licensure in the State of Florida.

Work Schedule:

7 Days On / 7 Days Off - Overnight Shift
