WAGE RANGE: $48.21 TO $64.50 PER HOUR

POSITION OBJECTIVE:
As a member of the patient care team, you will apply your knowledge, skills and professional judgement to support the provision of safe and effective medication-related care for patients of Niagara Health.

Responsibilities will include, but are not limited to, identifying and addressing patient-specific drug related problems, collaborating with prescribers and the interprofessional team in the optimization of medications, clinical review and verification of prescriber orders, supporting the Medication Reconciliation process, engaging in various quality improvement/safety initiatives or projects. The pharmacy program is committed to supporting a learning organization culture. As such, you will act as a preceptor to both pharmacist students and interns. You will work within your scope of practice within the hospital environment that is guided by policy and the organizational pharmacy practice model expectations. As a pharmacist at NH, your work will align with organizational core values, hospital pharmacist standards of practice and organizational priorities.

Foundational to the medication use process is also the safe and efficient dispensing of medications. As a pharmacist, you will have thorough knowledge of all dispensing workflows to be able to confidently oversee the accurate dispensing of medications, provide direction to pharmacy technicians if needed, and ensure expected policies and standards are met.

QUALIFICATIONS:
o BSc Pharmacy or PharmD degree
o Current registration with the Ontario College of Pharmacists in part A of the register, in good standing
o Hospital pharmacy residency required; or related clinical experience considered
o Demonstrated ability to prioritize patient care responsibilities
o Flexible, self-directed and able to work effectively with minimal supervision
o Demonstrated effective application of knowledge, skills and judgment as it relates to medication management
o Previous experience with the Cerner hospital information system
o Evidence of professional development and continuing education
o Excellent communication and interpersonal skills
o Demonstrated leadership qualities
o Demonstrated ability to communicate and work effectively within a team
o Consistent and regular attendance required
