Job Description

Provides professional, timely, patient-oriented pharmaceutical services in any of the Pharmacy service areas. Directs the activities of personnel assigned to the service area. Participates in quality assessment and continuous quality improvement activities. Complies with all appropriate safety and infection control standards. Participates in education and training functions. Assists in improvement and development of pharmaceutical services. Performs all job duties and responsibilities in a courteous and customer-focused manner according to the Hurley Family Standards of Behavior. Works under the general supervision of a Pharmacy Supervisor, Clinical Coordinator, or designee. Exercises working supervision, directs, and assigns the work of Pharmacy Technicians, Interns, and other support personnel assigned to the service area.

Responsibilities

  1. Reviews medication orders received for completeness and legality; verifies appropriateness of dose prescribed, route, frequency, and duration of therapy; screens for physical/chemical incompatibilities.
  2. Compares orders received against patient medication profiles to identify potential therapeutic problems, including allergies, drug duplication, interactions, and other contraindications.
  3. Personally prepares or directs the preparation of medication orders according to standards and protocols.
  4. Performs final quality control check of all completed medication orders.
  5. Calculates medication dosage and dosing intervals based on serum drug levels and patient disease/organ function status, according to pharmacokinetic principles.
  6. Conducts monthly Nursing Unit reviews to assure proper storage and use of medications and compliance with legal and other regulatory standards.
  7. Conducts information review, and education activities for patients, physicians, nursing staff, students and other Pharmacy personnel.
  8. Provides information to physicians, nurses, health care professionals and the public on the treatment of acute toxic or suspected toxic ingestions and drug therapy.
  9. Maintains necessary records for the service area, such as drug distribution, charge information, controlled substance dispensing, and drug pre-packaging records.
  10. Assists with the order, receipt, monitor, control and storage of pharmaceuticals.
  11. Participates in the development, review, maintenance and enforcement of the Formulary of Accepted Drugs.
  12. Participates in drug utilization review activities, medical audits, and medical care evaluations.
  13. Demonstrates effective judgment and ability to understand, react effectively and treat (if appropriate) the unique needs of patient age groups served.
  14. Adheres to sterile, non-sterile, and hazardous drug preparation guidelines in compliance with medical center policies and regulatory agency requirements.
  15. Performs other related activities/services as assigned. Utilizes new improvements and/or technologies that relate to job assignment.


Qualifications

  • Baccalaureate (B.S.) or professional (Pharm. D.) degree from an accredited School or College of Pharmacy.
  • Sufficient strength to perform moderate lifting.
  • Ability to stand for long periods of time.
  • Ability to maintain harmonious working relationships with patients, physicians, medical staff, vendors, and the general public.
  • NECESSARY SPECIAL QUALIFICATIONS:
  • Michigan license as a Registered Pharmacist and possession of a valid Michigan Controlled Substance license or eligibility for reciprocity for Michigan license as a Registered Pharmacist. Candidates hired as result of eligibility for reciprocity must obtain Michigan license as a Registered Pharmacist and a Michigan Controlled Substance license within six months of appointment. Failure to obtain the required license within the specified time frame will result in termination of employment.
