Summary:This ambulatory Dermatology-Gastroenterology position is responsible for the delivery of ambulatory care clinical pharmacy services and initiatives that optimize the patient care experience and drive organizational performance in the clinic setting. The role will focus on patient-centered care and supporting medication management, therapy optimization, and certain operational aspects of pharmacy care delivery.
Responsibilities:- Provide support for and maintain an ambulatory clinical pharmacy practice within the dermatology and gastroenterology clinics. May also provide coverage across additional ambulatory clinical practice areas, including, but not limited to: rheumatology, hepatology, and cardiology.
- Perform direct patient care duties related to medication management, consultation, and patient education through collaborative pharmacy practice protocols.
- Drive health-system pharmacy initiatives (i.e. biologic stewardship), cost effective therapy, and promotion of start to finish pharmacy care delivery.
- Maintain accountability for utilization of UCHealth outpatient pharmacies, focusing on medications requiring specialty pharmacy services.
- Serve as the medication expert for patients, pharmacists, and other health care professionals.
- Maintain a consistent patient care experience across multiple clinic settings.
This position is an onsite role and does not offer a hybrid or remote option
Minimum Requirements:
Bachelor's Degree in Pharmacy from an accredited school of Pharmacy.
State licensure as a Pharmacist and any relevant life support certification as determined at position level.
Clinical Pharmacist:
- Ensure medications are prescribed, accurately dispensed, and administered according to best practices
- Provide drug information and counseling to patients and caregivers regarding medication use, accurate dosage, potential side effects, and administration instructions
- Stay current with updated medication formularies, new drug therapies, treatment protocols, and emerging medical research through continuous professional development and clinical education
- Communicate with physicians, nurses, and other healthcare professionals to facilitate collaborative care to ensure optimal outcomes
