Job Summary
The Clinical Pharmacist will be responsible for reviewing and addressing reported side effects, handling requests for new orders, addressing therapy concerns, and troubleshooting infusion-related problems. Additionally, this role will involve conducting care plan reviews and mentoring and supervising intern pharmacists. The Clinical Pharmacist will report directly to the Pharmacist in Charge.

Duties and Responsibilities
• Supervise pharmacists and intern pharmacists including mentoring, coaching, and managing according to company policies.
• Communicate with medical professionals regarding patient status and response to medication, and when necessary to resolve patient issues.
• Create, review, and complete reassessments per Pharmacy protocol including conducting routine patient profile monitoring.
• Review and update Care Plans prior to each medication dispensing; assist in reviewing and discontinuing therapies for clinical appropriateness; troubleshoot infusion-related problems; and provide patient counseling when required.
• Create and review Proactive Risk Assessments.
• Participate in surveys conducted by authorized inspection agencies.
• Pursue continuing education appropriate to job responsibilities and license requirements.
• Report any misconduct, suspicious or unethical activities to the Compliance Officer.
• Represent the company in a professional and courteous manner in all job-related interactions.
• Ensure ongoing compliance with all laws and regulations and accreditation standards upheld by the company.
• Perform other duties as assigned by supervisor.

Education and Experience Requirements
• Clear and Active California Registered Pharmacist License.
• B.S. in Pharmacy.

Preferred Qualifications
• PharmD.
• Ig Certified Pharmacist (IgCP®), Certified Specialty Pharmacist (CSP).
