Summary:The primary care pharmacist will initiate and develop comprehensive medication management services within the primary care setting. This position is an integral member of the health care team, working collaboratively with other primary care providers, medical assistants, care managers, nurses, social workers and family members to ensure the delivery of quality, efficient, and cost-effective healthcare.
Responsibilities:- Manages individual patients and patient populations that have complex and/or multiple medication needs for acute and chronic disease states.
- Monitors drug therapy to evaluate appropriateness of use, dose, dosage form, regimen, route, therapeutic duplication, adherence, and drug interactions.
- Works with the primary care team and specialty providers under a collaborative agreement to assist with disease state management.
- Identifies, prevents and resolves drug related problems including adverse drug reactions and polypharmacy that could lead to adverse outcomes.
- Consults with medical providers on optimal drug therapy and drug therapy management.
- Establish productive, collaborative relationships with other healthcare professionals, focusing upon the attainment of organizational goals and objectives through a commitment to teamwork.
- Reviews medications of discharged patients from regional hospitals or skilled nursing facilities to ensure safety of high-risk populations.
- Provides patients and/or families with focused teaching on safe and effective use of prescribed medication.
- Document all activities in the medical record in accordance with Northern Light Health standards.
- Administers immunizations as needed.
- Participates in projects, workgroups and committees to improve patient outcomes.
- Complies with standards set forth by Northern Light Health and other regulatory bodies.

- Self-directed, motivated and capable of supervising support personnel.
- Graduation from an accredited school of pharmacy.
- Completion of PGY1 Residency Program or 3+ years of clinical pharmacy experience required.
- PGY2 Residency Program, Board of Pharmacy Specialties Certification, or 3+ years of ambulatory pharmacy specific clinical experience preferred.
- Pharmacist License in good standing and certification to administer drugs and vaccines in Maine by the Maine Board of Pharmacy required.
- Will have up-to-date training and certification in Healthcare Provider Basic Life Support issued by American Heart Association, American Red Cross or American Health & Safety Institute. Must obtain within 3 months if not current at hire.
- Board of Pharmacy Specialties Certification in Ambulatory Care Pharmacy (BCACP) preferred.
