DescriptionESSENTIAL JOB DUTIES:• Conduct required in-home supervisory visits per NYSDOH regulations and agency policy.
• Assess patient needs, environment, and Plan of Care implementation during supervisory visits.
• Evaluate Home Health Aide and personal care worker performance and competency in the home.
• Identify and address clinical concerns, care gaps, and safety issues; communicate findings to the care team.
• Provide on-the-spot coaching and training to field staff during visits.
• Document supervisory visits accurately and timely in the EMR.
• Respond to clinical questions and incidents from field staff, patients, and families.
• Collaborate with intake, scheduling, and operations on case starts, transitions, and care plan updates.
• Participate in on-call rotation as scheduled.
• Support the agency's PI/CQI program through chart audits and process improvement activities.
• Other duties as assigned by leadership.
AGE RELATED SKILLS & COMPETENCIES:• Oversees care delivered to geriatric, adult, and (where applicable) pediatric patient populations.
• Ensures field staff competency in age-appropriate care techniques and communication.
• Models culturally sensitive, age-appropriate interactions with patients, families, and staff.

RequirementsQUALIFICATIONS:• Active NY RN license in good standing required.
• Associate's degree in Nursing required; BSN preferred.
• Minimum 2-3 years of nursing experience required; home care and supervisory experience strongly required. Pediatric experience required.
• Current CPR/BLS certification.
• Strong knowledge of NYSDOH home care regulations and supervisory visit requirements.
• Excellent assessment, coaching, and communication skills.
• Proficiency with electronic medical records; HHAeXchange preferred.
Knowledge of UAS system prefered
• Reliable transportation, valid NY driver's license, and proof of auto insurance.
PHYSICAL ACTIVITIES:• Frequent standing, walking, bending, stooping, kneeling, and reaching during patient visits.
• Lifting and carrying nursing supplies and equipment up to 25 pounds; occasional assistance with patient transfers up to 50 pounds.
• Manual dexterity required to perform clinical procedures, administer medications, and operate medical equipment.
• Ability to drive between patient homes and to and from the office throughout the workday.
• Climbing stairs may be required to access patient homes.
