Who You Are as a Nurse Practitioner:• A compassionate and patient-focused Nurse Practitioner
• Passionate about improving quality of life for patients with serious illnesses
• Experienced in palliative care, hospice, home health, geriatrics, or chronic disease management
• A strong communicator who values teamwork and patient advocacy
• Licensed and board certified as a Nurse Practitioner in Pennsylvania
What You'll Do as a Nurse Practitioner:• Provide comprehensive palliative care assessments and symptom management
• Support patients and families through goals-of-care discussions and advanced care planning
• Collaborate with physicians, nurses, social workers, and interdisciplinary team members
• Manage complex and chronic conditions with a focus on comfort and quality of life
• Educate patients, caregivers, and staff on palliative care services
