Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join our established
Palliative Care team at

Position Highlights - Join an established hospital-based Palliative Care program
- Monday-Friday schedule, 8:00 AM - 5:00 PM
- No procedures required
- Collaborative team environment with experienced physicians and clinical staff

Responsibilities - Provide comprehensive palliative care consultations and follow-up care for hospitalized patients
- Collaborate with physicians, nurses, case management, social work, chaplain services, and other interdisciplinary team members
- Assist patients and families with goals-of-care discussions, advance care planning, and symptom management
- Develop individualized treatment plans that align with patient values and preferences
- Document patient care accurately within the electronic medical record
- Participate in multidisciplinary rounds and care coordination
- Licensed Nurse Practitioner or Physician Assistant (or eligible for Florida licensure)
- Board Certified
- Interest or experience in Palliative Care and/or Hospice Medicine
- New graduates are welcome to apply if they demonstrate a strong interest in palliative medicine
- Excellent communication and interpersonal skills
- Ability to work collaboratively in a multidisciplinary team
