Responsibilities

  • Work Monday to Friday, 8:00am to 5:00pm—no weekends or holidays!
  • Collaborate with a dedicated team, including support staff and various therapists
  • Manage a small patient volume of 5 patients per day
  • Participate in on-call duties approximately once every 7 weeks during off hours
  • Build meaningful relationships with elderly participants in person-centered care

Responsibilities

You Can Expect:
  • Monday - Friday 8:00am - 5:00pm - no weekends or holidays!
  • Dedicated team of providers, support staff, physical therapists, occupational therapists, dieticians, and social workers
  • 5 patients per day - small volume
  • Call is once every 7 weeks, 5pm - 8am, you are not expected to go out, calls are triaged before reaching you!

Qualifications

  • Graduate of an accredited Nursing Program, and Masters of Science Degree in Nursing (MSN)
  • 2 years of Nurse Practitioner experience
  • Current licensure by the Virginia State Board of Nursing as a Nurse Practitioner with prescriptive authority. Maintains BLS and ACLS Certifications. Hold a current, active license as a registered nurse in Virginia or hold a current multistate licensure privilege as a registered nurse. Nationally recognized certification in field of specialization
  • Experienced providers in palliative, hospice, primary care, or long-term care are encouraged to apply
