Qualifications

• Doctor of Pharmacy (PharmD) or Bachelor's of Science in Pharmacy (prior to 2006) required.
• Successful completion of an ASHP accredited PGY1 or PGY2 residency may be required.
• 0-3 years of relevant pharmacy experience needed.
• Current Indiana Pharmacist license or eligibility required.
• Board certification or specialized training may be needed.
• Basic Life Support (BLS) certification through American Heart Association typically required.
• Willingness to travel to IU Health facilities may be necessary.

Responsibilities

• Provide comprehensive, value-based medication-related care to improve patient quality of life.
• Collaborate with healthcare professionals to ensure effective medication therapy outcomes.
• Process and dispense medications according to orders or prescriptions.
• Monitor medication therapy and ensure regulatory compliance.
• Maintain security of medications and protect patient confidentiality.
• Offer medication information support to medical and nursing staff.
• Identify and intervene on medication-related issues to ensure continuity of care.
