Qualifications

• Licensed in Social Work or Counseling (LISW, LCSW, LPC, LMFT, etc.)
• Experience working with older adults (65+)
• Strong documentation skills in compliance with Medicare guidelines
• Ability to work collaboratively within a small, multidisciplinary team
• Commitment to enhancing patients' quality of life through kindness and accountability

Responsibilities

• Collaborate with a psychiatric physician, RN Director, and medical office coordinator
• Develop, personalize, and update treatment plans using evidence-based approaches
• Document patient care in compliance with Medicare within set time-frames
• Facilitate group therapy as the primary treatment method
• Conduct individual and family therapy sessions as needed
• Assist with community education initiatives
• Perform other assigned duties as necessary
