How you'll contribute A Outcomes Manager who excels in this role:
- Coordinate PPD data collection with the interdisciplinary team.
- Ensure supportive documentation for QIs is complete in the medical record.
- Complete admission and discharge IRF-PAIs within CMS-designated timeframes.
- Abstract required information from medical records and summarize on the IRF-PAI.
- Enter data into the CMS-accepted software/program within designated timeframes.
- Maintain and update the IRF-PAI log; verify accuracy against the patient location roster.

What we're looking for:- Clinical degree with current license to practice as an OT, PT, RN, SLP, or MSW
- QI competency required
- Working knowledge of state and federal guidelines; familiarity with healthcare workflows and documentation required
- Basic computer and data entry proficiency required
- Strong clinical assessment/analysis, effective verbal and written communication, excellent interpersonal skills, and ability to collaborate across disciplines
