Oncology Clinical Pharmacist

Position Summary

Under supervision, and according to state and federal law covering the practice of pharmacy, establishes policies and procedures; compounds and dispenses medications and other pharmaceuticals supplies. Provides a pharmaceutical service and education for a patient population that includes ages ranging from newborn through geriatric. Works with multi-disciplinary health care team in the oncology setting.

Responsibilities

  • Provide clinical pharmacy services for outpatient oncology population.
  • Support operational needs of oncology pharmacy department.
  • Communicate with and assist pharmacy management on oncology related initiatives.
  • Monitor patient medications to determine possible medication therapy problems.
  • Verify oncology physician orders and follow protocols/procedures.
  • Dispense chemotherapy medications and manage technician refilling of automated dispensing cabinets.

  • Supervise technician and/or prepare chemotherapy admixture solutions according to physician orders.

  • Supervise pharmacy technicians on duty for accuracy of work.

  • Provide clinical guidance to multi-disciplinary staff pertaining to medications as needed for oncology patients.
  • Provide drug information as requested to physicians, nurses, and patients.
  • May cross-train to inpatient pharmacy as needs and candidate aptitude allow.


Requirements

  • Pharm D or Bachelor's degree in pharmacy.
  • Licensure as a Pharmacist in the State of Ohio.
  • Prior oncology experience strongly preferred.
  • ACLS certification required (opportunity to obtain this post-employment will be provided if needed).
  • PALS certification preferred (opportunity to obtain this post-employment will be provided if needed).


  • Specific disease state and/or Board of Pharmacy Specialties certification preferred (opportunity to obtain this post-employment will be provided if needed).
  • Opportunity to cross-train to other pharmaceutical specialties (EG, Emergency Medicine, Critical Care Medicine, Medication Therapy Management) will be provided as department needs and pharmacist aptitude allow.
  • LMH is accredited by DNV and TJC, and as such, may require specific annual education related to specialty certifications and standards.
