Halcyon Home is hiring an
In-Office Home Health RN Case Manager to support our growing geriatric population in
Austin, TX. We offer competitive pay,
5 weeks of PTO plus 9 holidays per year, and wonderful team support!
Key Responsibilities:- Supervise and support clinical staff including nurses and therapists
- Review and approve patient assessments, care plans, and documentation
- Monitor quality assurance and performance improvement activities
- Assist with staff training, scheduling, and performance evaluations
- Support survey readiness and audit preparation
- Address clinical concerns and provide problem-solving support to staff
- Ensure compliance with Medicare, Medicaid, state, and accreditation standards
- Coordinate patient care and clinical services with physicians and interdisciplinary teams

Requirements
- Graduate of an accredited nursing program (Diploma, ADN, or BSN)
- Active, unencumbered RN license (state or compact/NLC license)
- Minimum 2 years of clinical nursing experience and 1 year of home health experience
- Management experience preferred
- Current driver's license, CPR certification, and reliable transportation with auto insurance

Salary Description
$80,000 to $90,000 per year
