Halcyon Home is hiring an In-Office Home Health RN Case Manager to support our growing geriatric population in Austin, TX. We offer competitive pay, 5 weeks of PTO plus 9 holidays per year, and wonderful team support!

Key Responsibilities:
  • Supervise and support clinical staff including nurses and therapists
  • Review and approve patient assessments, care plans, and documentation
  • Monitor quality assurance and performance improvement activities
  • Assist with staff training, scheduling, and performance evaluations
  • Support survey readiness and audit preparation
  • Address clinical concerns and provide problem-solving support to staff
  • Ensure compliance with Medicare, Medicaid, state, and accreditation standards
  • Coordinate patient care and clinical services with physicians and interdisciplinary teams

Requirements

  • Graduate of an accredited nursing program (Diploma, ADN, or BSN)
  • Active, unencumbered RN license (state or compact/NLC license)
  • Minimum 2 years of clinical nursing experience and 1 year of home health experience

  • Current driver's license, CPR certification, and reliable transportation with auto insurance
