We are currently looking for an Occupational Therapist to join our team on a full-time basis. The position would be responsible for providing occupational therapy evaluation and treatment to patients in their homes or alternate community settings within Monmouth County, NJPosition Summary:Responsible for providing occupational therapy evaluation and treatment to patients in their homes or alternate community settings
Principal Responsibilities:- Support organization's mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers.
- Assess and evaluate clients for occupational therapy needs and develops and implements appropriate plan of action in collaboration with the physician
- Document all changes in client's condition and updates care plans and outcomes accordingly
- Identify patient, caregiver, and family counseling and education needs and plans appropriate teaching strategies
- Monitor quality and appropriateness of services provided
- Participate in VNAHG's QAPI and in-service programs
- Adhere to agency infection control and safety policies
- Adhere to the organization's policy in regard to absenteeism and appearance
Job Competencies & Minimum Qualifications:
- Occupational Therapy Licensure in NJ
- 2-3 years of prior clinical experience and community-based experience preferred
- Strong computer skills
- Strong communication and interpersonal skills
- Valid NJ driver's license
Working Conditions/Physical Demand: Working indoors/outdoors, driving, lifting, sitting, standing, walking, fine motor hand movement, use of phone and tablet for documentation
