Description SUMMARY: Provides case management and direct patient care within a team environment. Administers comprehensive skilled care.
Schedule: Monday - Friday, 8 to 430pm No weekends, no holidays, No on calls

QualificationsEducation & Experience - Applicants must possess the following qualifications or an equivalent combination of education and experience:
- Education: Graduate of an approved Occupational Therapy program.
- Experience: Minimum of one-year experience in a hospital inpatient setting.
- Licensure: Possession of current OT license to practice in New York State.
Responsibilities - Provide skilled OT services to patients in their homes.
- Complete patient evaluations and develop individualized treatment and discharge plans.
- Deliver interventions to improve independence, safety, self-care, and daily living skills.
- Educate patients, families, and caregivers on treatment plans, adaptive strategies, and home safety.
- Monitor patient progress and modify treatment plans as needed.
- Collaborate with an interdisciplinary care team to coordinate patient-centered care.
- Supervise COTAs as applicable.
- Maintain timely, accurate clinical documentation.
- Communicate changes in patient condition and coordinate appropriate follow-up.
- Connect patients with community resources and support services.
- Participate in care conferences, education, quality improvement, and team initiatives.
- Maintain required productivity, quality, and regulatory standards.
Salary: $67,700 - $115,045.00
